Molekuljarno-genetic markers.

From 103 patients who have transferred earlier various kinds of a resection of a stomach concerning a cancer, at 40 (38,8 %) patients in the course of observation in the postoperative period besides standard researches in a material of a mucous stump of a stomach received at esophagogastroduodenoscopy defined molekuljarnogeneticheskie markers.

In work the panel of molekuljarno-genetic markers developed and tested in clinic of faculty surgery of a name of N.N.Burdenko which included definition of an abnormal methylation of genes CDHl, RASSF1A, MLHl, N33, DAPK and RUNX3 is used.

DNA methylation is a process of covalent joining metilnoj groups to tsitozinu as a part of CG-dinucleotide, kataliziruemyj DNA-methyltransferase enzyme. The methylation promoternyh districts of various genes leads transkriptsionnomu to gene silence, promoting a full inactivation of a gene expression [40; 86].

The abnormal methylation of genes-supressorov is extended epigeneticheskim process of an inactivation of genes in a tumour and can serve as a marker of tumoral growth. The abnormal methylation of genes-supressorov is one of early events of a carcinogenesis and can be taped long before clinical implication of tumoral growth. The abnormal methylation of genes CDH1, RASSF1A, MLH1, N33, DAPK, RUNX3 leads to a functional inactivation of these genes.

Gene CDH1 codes fiber E-kadgerin which is responsible for adhesion of cells. Depression of activity of a gene leads to loss of cellular contacts, and acquisition by a tumoral cell of ability to an invasion and an innidiation [101; 102; 104; 131].

Gene RASSF1А - a gene-supressor, participates in regulation of a cellular proliferation and in an apoptosis, codes the factor supervising a cellular cycle [111; 130].

Gene MLH1 - a gene-supressor, codes the factor participating in a reparation of not coupled bases, arising owing to errors replikatsii DNA [107].

Gene N33 shows supressornye functions and shows high level of a methylation in various types of tumours occurring of an epithelium [24; 40].

Gene DAPK codes albuminous kinazu, providing an apoptosis and preventing tumoral progression as a result of inhibition of polarisation of a cell. The inactivation of gene DaP-kinazy leads to intensifying of migration and an invasion of tumoral cells, interferes with an apoptosis [132].

Gene RUNX3 codes the fiber concerning family transkriptsionnyh of factors, containing the Runt-domain. Fiber function consists in formation of a complex which contacts a cow sequence of DNA which are in many enhansernyh and promotor areas of various genes that leads aktivirovatsii or to suppression of a transcription of these genes. This fiber also can co-operate and with others transkriptsionnymi factors, strengthening a gene expression. [125].

Thus, all listed genes possess supressornymi properties, immediately influencing on restraint of tumoral growth. Their inactivation by means of an abnormal methylation promotes tumour development. It is necessary to notice, that the inactivation of genes-supressorov is not taped at development of inflammatory diseases of various organs, and also at reversible hyperplastic processes. The abnormal methylation is defined at level of a dysplasia of 2-3 degrees and assotsiirovano with the beginning of tumoral growth that does the given molekuljarno-genetic markers high-strung for early definition of tumoral growth.
