Conclusions under the chapter II

Working out and approbation of algorithm of educational speech influence of the doctor on the patient, the description and which results are resulted in the second chapter, have allowed to come to following conclusions.

1) In the course of realisation of pedagogical educational activity, the doctor addresses the educational text - oral (or written) the speech product created by the doctor and addressed to the person, addressed for the medical aid, focused on formation at the patient of representations about sense of conducting zdorovesoobraznogo a way of life, treatment forecasts, principles and rules of behaviour in the conditions of disease and the treatment, consisting of a number of the specific educational speech messages logically among themselves connected, subordinates of the uniform purpose and possessing the certain stylistic colouring caused by features of the addressee, the sender, and also a situation of their interaction.

2) the Maintenance and a composition of the educational text have been defined on the basis of the analysis of the researches devoted to educational activity of the doctor and linguistic aspects of interaction of subjects of medical process. Substantially educational text includes following components:

- Bioethical component (connected with smyslozhiznennymi orientations of the patient to preservation of and the health);

- Biomedical making (including biomedical imperatives, immutable requirements which allow the person to keep the life and health);

- prognosticheskaja making (meaning scoring of prospects of the patient which wait for it in case of conducting zdorovesoobraznogo a way of life).

3) the Educational text consists of educational speech messages, each of which represents the statement or
Set of the statements bearing in ability to influence, capable to change the relation, vision, understanding of, another, the world in whole (in the given work it is a question of change of understanding by the person of the educational information, concerning features of a physical condition and the recommended model of behaviour connected with health). These statements can influence differently the patient and the validity surrounding it: to inform, convince, stimulate to performance of certain actions; on occasion the fact of their pronouncing can change definitely the validity, (for example, the treatment schedule).

As a theoretical substantiation of working out of pedagogical educational speech messages which would allow to change consciousness and behaviour of the patient, the theory of speech certificates - one of key directions pragmalingvistiki, studying questions of speech influence is chosen. On the basis of the analysis of substantive provisions of the theory of speech certificates and features of the educational activity realised by the doctor, following kinds of educational speech messages have been allocated:

- The speech messages of the doctor having informative character (apellirovanie to consciousness of the patient). The report of the certain information to the patient, its notification becomes the Main task of such kind of speech messages.

- The speech messages of the doctor having character of rendering of influence on behaviour (apellirovanie to consciousness and behaviour of the patient).

In a context educational intentsy the doctor, following types of educational speech messages can be allocated: descriptive statements (describing the validity of a psychosomatic condition of the patient and a situation of its treatment); deonticheskie statements (concerning permissions and prohibitions to the patient of those or other formats of behaviour); perlokutivnye statements (connected with achievement
perlokutivnogo effect - the possible influence rendered by the statement on the patient).

The analysis of the specified kinds of speech messages, and also parametres according to which this or that educational message can be carried to a certain class (messages with informative character, or messages with character of influence) has allowed to conclude the following. At drawing up of educational speech messages it is necessary to pay attention to an originality of lexical structure, in particular, to a choice of "verifying" verbal forms which can be used at the statement analysis.

The algorithm of forming of the educational speech messages, assuming is offered, that each educational speech message "follows" from previous and it is logical with it is connected.

4) Working out and approbation of the educational text have been made within the limits of interaction with the patients having chronic illness of kidneys of V stage, and receiving treatment by a hemodialysis (demanding from the patient of serious change of style of a life). Approbation of the educational pedagogical text focused on formation of representations about zdorovesoobraznom behaviour at patients with chronic illness of kidneys of V stage, receiving treatment by a hemodialysis, has been made as follows. Interaction with patients was carried out on the basis of joint model of the profession of a physician assuming partnership and parity relations. During educational interaction features of patients have been taken into consideration. For the purpose of more detailed adaptation of the text of educational conversation the information on following psychological features of patients has been used: Uneasiness levels, ekstraversija and an introversion, ways sovladajushchego behaviour and level of the subjective control.

Following the results of the given talks the doctor made an estimation of level of mastering of the knowledge received by the patient. Studying of level of representations about
Conducting regulations zdorovesoobraznogo a way of life, generated after carrying out of pedagogical educational conversation, it was realised by means of specially developed questionnaire. It has been revealed, that in experimental group the quantity of true answers was more than in control. At the same time, the disorder was small, that could testify to enough adequate representations at patients of control group about regulations zdorovesoobraznogo behaviour.

During a control stage of experiment supervision over behaviour and a condition of patients of experimental and control groups in dynamics (within one year) was carried out. The analysis of data from the case record (the filled doctor) has allowed to reveal, is how much effective at patients skills zdorovesoobraznogo behaviour have been generated. The analysis of behaviour of patients within a year was carried out in the basic directions:

1.reguljarnost visitings of procedure of a hemodialysis;

2. Results of analyses of blood on which it is possible to judge a correct mode of reception of medicines;

3. Results of measurement of weight at each patient before hemodialysis procedure (according to which it is possible to judge observance by the patient of a diet and a vodno-drinking mode).

Regular visitings of a hemodialysis by the majority of patients and observance of other medical recommendations by the majority of patients of experimental group (93,7 %) have allowed to confirm that fact, that at bolshej parts of patients following the results of pedagogical educational conversation have appeared steady representations about smyslozhiznennyh aspects of preservation of health, about the importance of visiting of procedures, treatment regulations, and possible consequences of infringements of doctor's instructions.

Statistical processing of the data received during experiment has allowed to conclude the following. There is a direct interrelation between
Following factors: 1) understanding the patient of the importance of observance of regulations of conducting zdorovesoobraznogo a way of life and its adherence zdorovesoobraznomu to a way of life; 2) conducting by patients zdorovesoobraznogo a way of life and the positive compensated condition of their health; 3) level of understanding the patient of principles of conducting zdorovesoobraznogo a way of life in the conditions of replaceable therapy by a hemodialysis and a somatic condition of the patient.

By results of approbation of the developed algorithm of educational speech influence of the doctor on the patient it is possible to draw a conclusion of its ability to have necessary influence on the addressee of the educational text (patient), forming at it zdorovesoobraznoe behaviour that confirms a research hypothesis.
